When you first book an appointment with me, you will be sent a comprehensive questionnaire to complete prior to the consultation along with a food diary. You can also download them below, if you prefer. Please complete this questionnaire as fully as possible, as this will save time during the consultation, which usually takes about an hour. The food diary can be brought with you to the consultation and must be filled in for 3 days with as much information as possible. These will be reviewed at length during the consultation.
Having gained a full case history, I will then explain what I feel are some of the possible causes to your symptoms, and we can make changes to your diet together in which you feel are realistic and achievable to help alleviate these symptoms and improve your energy and vitality.
You will be able to take away with you from the consultation an action plan together with some general nutritional information. If I feel that supplements would be beneficial, I will make recommendations.
I will ask a health and nutrition questionnaire for the client to complete. An initial consultation typically lasts 60 to 90 minutes, and in this time I will ask detailed questions such as:
- current health concerns
- diagnosis and treatment
- medical history
- family history
- levels of physical activity
- use of medication and supplements
I will evaluate individual needs and uses the extensive evidence base for nutritional science to develop a personalised, safe and effective nutrition and lifestyle programme.
Follow up consultations are generally after four weeks in order to monitor progress and make any necessary adjustments. Further follow-ups may be required depending on each individual situation.
